St. Croix Valley Portico Healthnet
Affordable Healthcare for People Who are Uninsured St. Croix Valley (SCV) Portico Healthnet is designed to make healthcare affordable for people without insurance. Our goal is to make it easier for people to get the healthcare they need. Participants pay a small monthly fee and receive basic health and medical services. The mission of St. Croix Valley Portico Healthnet is to reduce the number of people without coverage for healthcare services.
How does it work?
You will choose one of the St. Croix Valley (SCV) clinics to be your primary care clinic. They are:
- Stillwater Medical Group (SMG)
- St. Croix Family Medical Clinic
This is where you will go for your healthcare needs.
SCV Portico Healthnet staff can also help you with any social or health concerns. As a first step, you will complete a Family Health Assessment with a Care Management Coordinator. If you have any concerns, the Care Management Coordinator can help identify community resources that may be able to help. Your Care Management Coordinator is available to meet with you regularly and can answer any questions that you have about the program.
What services does St. Croix Valley Portico Healthnet offer?
The program pays for primary and preventive care services.
Examples of these services are:
- clinic visits for routine physicals, well child visits,
- immunizations and care when you are sick
- prescription medications
- outpatient mental health services
- eye exams
- interpreter services
- limited specialty services
- care management services
What does it cost to participate in St. Croix Valley Portico Healthnet?
St. Croix Valley Portico Healthnet enrollees pay a small monthly participation fee based on income and family size.
Who qualifies?
To be eligible for enrollment in SCV Portico Healthnet, participants must meet all of the following criteria:
not currently insured
not already eligible for Medical Assistance, General Assistance Medical Care or Medicare
resident of Washington County who uses Lakeview Hospital as their primary community hospital
meets the income guidelines
